New Patient Registration

 

If you are interested in registering with this practice, you will need to either complete an online application form or alternatively collect one from the practice.

How to Register

Our practice covers Mawsley, Broughton, Kettering, Barton Seagrave, Burton Latimer, Rothwell, Desborough, Cransley, Pytchley, Walgrave, Old, and other surrounding villages.

If you live within the boundary area, please complete our new patient registration.

When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.

Your Named GP

From the 1st April 2015 the government have added to our NHS contract that all patients should have a named accountable GP. We have now allocated a named accountable GP to all our patients . If you wish to know who your named accountable GP is please contact the Surgery.

The named accountable GP to whom you are allocated will take responsibility for the co-ordination of all appropriate services and ensure they are delivered where required to each of their patients. However there is no condition within these regulations for you to see your named GP when you book an appointment at the Surgery, so your overall care will not be affected whichever GP you prefer to see.

If you feel you would like a different GP allocated to you then please let us know and we will do our best to accommodate this.

Any patient awaiting Weavers Medical registration can be seen as an emergency, if required, if an appointment is available.

Temporary Registration

We are always happy to see any relatives or friends staying with you if they need a doctor or nurse.  They will need to fill out a short form for the information to be returned to their own doctor.  Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
